Welcome to our PSC Partner Page
Our commitment has always been to aid in the protection of our collective communities. We developed this page to give our partners easy access to sample questionnaire templates, various state release forms for criminal history repositories and motor vehicle records, and law enforcement supplemental forms. For our full background investigation clients, we offer a place to upload completed forms for us to coordinate our work assignments.
We define "partner" as Tribes that are our current clients as well as attendees to our training programs. The forms are for your information. Please see the disclaimer below for any questions about the use of the provided templates.
